Information about billing, insurance coverage, payment options, and important patient policies at Keady Family Practice in Claremont, NH.

Insurance & Coverage

Keady Family Practice works with many major insurance providers to help patients access the care they need. Coverage and benefits vary by plan, and patients are responsible for applicable copays, deductibles, or balances not covered by insurance.

Online Bill Pay

For your convenience, patients can securely pay their medical bills through our online payment portal.
 

Online bill pay allows you to:

• View your balance
• Make secure payments
• Pay using a credit or debit card
• Access your account anytime

Changing Scheduled Appointments Policy

To ensure appointment availability for all patients, Keady Family Practice requires advance notice for cancellations or rescheduling.
 

Advance Notification

All scheduled appointments require at least 24 hours notice if you need to cancel or reschedule.
 

Appointments canceled or rescheduled with less than 24 hours notice will incur a $75 missed appointment fee. Insurance companies do not reimburse this fee.
 

Weekend and Monday Appointments

Appointments scheduled for Saturday or Monday must be canceled by 9:00 AM on the Friday before the appointment to avoid a cancellation fee.
 

Dr. Keady Appointments

Patients scheduled with Dr. Cecilia Keady must follow all appointment policies. Repeated violations may result in forfeiting Dr. Keady as your primary care provider, and you may be reassigned to another provider within the practice.
 

Repeated Missed Appointments

Patients who miss or reschedule appointments three times within a 12-month period may no longer be eligible to schedule appointments and may be required to find another primary care provider within 30 days.
 

Missing an evening or weekend appointment may result in appointments being limited to 9:00 AM – 3:00 PM for one year.


Patients may also be required to keep a credit card on file to cover missed appointment fees.
 

Unpaid Fees

Missed appointment fees must be paid within 30 days of receiving a statement. Unpaid fees may be sent to collections after the 30-day grace period.

Practice Policies

Keady Family Practice is committed to providing fair, respectful, and accessible care for all patients. The policies below outline important information about patient rights, financial assistance programs, and our commitment to equitable healthcare.

Sliding Fee Discount Program

Information about financial assistance options available to eligible patients.
 

No one will be denied access to services due to an inability to pay. A discounted/sliding fee schedule is available based on family size and income.
 

Patients who may have difficulty paying for services are encouraged to apply for financial assistance through our Sliding Fee Discount Program.

Non-Discrimination Policy

Keady Family Practice provides services without discrimination based on race, color, national origin, age, disability, or sex.

Identification Policy

Patients may be asked to provide a valid photo ID and current insurance card during visits to ensure accurate patient records and proper billing.

Controlled Medication Policy

Rarely will any of our providers prescribe narcotic medications.

Keady Family Practice reserves the right to refuse service to individuals seeking narcotic or pain medications.

Medical Records Access

All patients of Keady Family Practice have the right to access their medical records electronically or in printed form.
 

Medical record requests will be fulfilled within 30 days.
 

Patients are encouraged to access their records through the secure patient portal whenever possible.
 

Printed medical records exceeding 30 pages may be subject to a $15 printing fee.

Patient Relationship Policy

All patients who utilize services at Keady Family Practice accept the terms and policies of the practice whether or not a policy agreement form has been signed.
 

Use of services establishes a primary care relationship with Keady Family Practice unless the patient requests that their records be transferred to another provider.

Emergency Care Disclaimer

Keady Family Practice LLC and FastMed Walk-In Clinic provide primary care services only and are not emergency care facilities.
 

We do not replace an emergency room or urgent care center.
 

If you are experiencing a life-threatening illness or injury, please call 911 or go to the nearest emergency room immediately.
 

Patients must have a scheduled appointment and be established with Keady Family Practice to receive services.
