****Keady Family Practice will always serve patients regardless of their ability to pay. We will not refuse service because of monetary issues if a patient shows up for a scheduled appointment. Patients with an inability to pay are required to apply for financial assistance for continued care.****

All patients of Keady Family Practice have the right to access their medical records electronically and/or physically. A patient can request for their records to be printed off in entirety or partially and Keady Family Practice will compile within 30 days. However, we do insist on ecological reasons to utilize the electronic version of medical records available through the portal. We will also encourage you to print them off at your own house from your portal. If you do insist on your medical record to be printed we will charge a $15 fee for any medical records that exceed 30 pages.****
****All patients that utilize Keady Family Practice for any services are accepting the terms of use and all policies of Keady Family Practice regardless of whether they have signed a policy agreement form. The use of Keady Family Practice services creates a primary care relationship with the patient, regardless of the status of the record transfers. The mere use of our facility constitutes the transfer of primary care according to our policies. All people seen at our facility are our primary care patients until they indicate otherwise or the patient requests their records to be transferred to another facility.****

You must be a patient of Keady Family Practice to utilize any of the services of Keady Family Practice or FastMed Walk-in Clinic. You must have a scheduled appointment to be seen by any of our providers or staff members in a medical capacity. The facilities, grounds, and surrounding areas under the control of Keady Family Practice LLC. are not open to the public and only open to patients and whom they have allowed coming with them.***
